The Community Solutions Hub will centralize fragmented Washington County data from diverse social institutions in a convenient location to measure quality of life for residents, monitor progress in their strengths and need, and ultimately catalyze precise action to achieve community well-being.

This free and powerful tool managed by San Mar Family & Community Services, and Meritus Health has so many opportunities, including: 

• Comprehensive neighborhood data points
• Direct linkages and contact information to related best practices
• Funding opportunities to help pay for those practices
• Report generation to help communicate strengths and needs, plus opportunities
• Online data portal with more than 200 social, economic, and health indicators
